What is Specialist Support Coordination?
Specialist Support Coordination is included in a plan when someone’s situation is more complex and needs a higher level of skill. It is time-limited. The focus is on reducing barriers, making the support environment safer and clearer, and connecting you with the right services.
It can include an intervention plan that disability or mental health workers then help to carry out.
Our Specialist Support Coordinators include tertiary qualified professionals with experience in complex case management. We work with people across a range of disabilities, including autism, acquired brain injury, psychosocial disability, and dual disability.
